The Shift

Terms of use

Last updated October 2, 2026

The Shift is a game for practicing nursing judgment. It's here to help you learn, not to tell you how to care for a real patient.

The short version

Agreeing to these terms

By playing The Shift, using its website, instructor dashboard or review pages, or creating an account, you agree to these terms. If you don't agree, don't use The Shift. "We" and "us" mean The Shift and its creator.

An educational simulation only

The Shift is a game for practicing clinical judgment. It is provided for general education and entertainment. It is not medical, nursing, pharmacy, legal or other professional advice, and using it does not create any nurse–patient, provider–patient or other professional relationship.

It is not a substitute for an accredited nursing program, clinical instruction, preceptorship, continuing education requirements, your licensing board's rules, your employer's or school's policies, or the judgment of a licensed professional.

Not for patient care

Do not use anything in The Shift to make decisions about the care of a real person. Medication doses, protocols, nomograms, lab values and thresholds, scoring, timing and the legal framework are simplified and adapted for teaching. They may differ from current clinical guidelines, manufacturer labeling, your facility's protocols and the law where you live or work.

In real practice, follow the orders you're given, your facility's policies and procedures, current drug references and the scope of practice in your state. If you have a question about your own health, talk to a qualified health professional. In an emergency, call 911.

Accuracy and clinical review

Content is written to reflect commonly taught nursing practice as of its last update, and each shift shows whether it has been reviewed by a clinician. Even so, medicine and law change, reviewers can disagree, and errors happen. We don't promise that any content is accurate, complete, current or suited to your situation, and we may change or remove content at any time.

If you spot something clinically wrong, please tell us through Menu → Send feedback or report a problem.

Laws and settings vary

The psychiatric shifts are written to California law (for example, 5150, 5250, 5585, Riese and LPS conservatorship) as one example. Other states' laws differ. A General US setting replaces those terms with plain descriptions, but neither version is a statement of the law where you practice. Facility policies also differ from unit to unit.

Exams and trademarks

The Shift is not affiliated with, sponsored by or endorsed by the National Council of State Boards of Nursing (NCSBN). NCLEX®, NCLEX-RN® and NCLEX-PN® are registered trademarks of NCSBN, used here only to describe the exam the practice relates to. Practice questions are original and are not actual exam items. Using The Shift does not guarantee that you will pass any exam, course or competency check.

Sensitive content

Shifts depict suicide risk, self-harm, child abuse, overdose, alcohol withdrawal, physical restraint, serious illness and death, written at the level a nurse would encounter them. Take a break whenever you need to. If you are thinking about harming yourself or are in crisis, call or text 988 (Suicide & Crisis Lifeline, US), or call 911.

Who it's for

The Shift is intended for adults (18 or older) in nursing education or practice. Younger learners may use it only with a parent's or educator's permission and supervision.

Fictional characters

All patients, families, staff, hospitals, units and events in The Shift are fictional. Any resemblance to real people or events is coincidental. Please don't enter real patient information or anything that identifies a real person anywhere in the game, including feedback.

Your account, purchases and feedback

You're responsible for activity on your account. If you buy the shift pack, payment is handled by Stripe; contact us if something goes wrong with a purchase. Feedback and clinical review comments you send may be used to improve The Shift without any obligation or payment to you. How data is handled is described in the Privacy page.

No warranties

The Shift is provided "as is" and "as available", without warranties of any kind, express or implied, including warranties of accuracy, fitness for a particular purpose, merchantability, non-infringement, or that it will be uninterrupted or error-free, to the fullest extent permitted by law.

Limitation of liability

To the fullest extent permitted by law, The Shift and its creator, contributors and clinical reviewers are not liable for any direct, indirect, incidental, consequential, special, exemplary or punitive damages, or for any loss of data, income or opportunity, arising out of or related to your use of, or inability to use, The Shift or any decision or action taken based on it, even if advised of the possibility of such damages. If any liability can't be excluded, it is limited to the amount you paid for The Shift in the 12 months before the claim, or US $50 if you paid nothing.

You agree to use The Shift at your own risk, and you are solely responsible for how you apply anything you learn from it. Some jurisdictions don't allow certain limitations, so some of the above may not apply to you.

Indemnity

To the extent permitted by law, you agree to hold harmless The Shift and its creator from claims arising out of your misuse of The Shift, including using it to make decisions about real patient care, or your breach of these terms.

Changes and general terms

We may update these terms. When they change in a way that matters, the date above changes and the game asks you to agree again. These terms are governed by the laws of the State of California, without regard to conflict-of-law rules. If any part is found unenforceable, the rest still applies.
